Please complete this form after each authorized family or client visit. It documents the family’s reported needs, spiritual-care concerns, relevant history, your direct observations, and recommended follow-up for Emerge Ministries’ leadership and case-management team.

Provide clear, factual, and relevant information while avoiding unnecessary confidential details, speculation, or unsupported conclusions. Most reports should take approximately 15–20 minutes to complete.

I certify that I have signed the required volunteer waiver and have been authorized by the head chaplain representing Emerge Ministries at this disaster site to minister to this family or client and to complete and submit this report as an Emerge Ministries volunteer. I understand that this authorization does not permit me to make promises, financial commitments, or guarantees of assistance on behalf of Emerge Ministries.
